California Privacy Policy
This CALIFORNIA PRIVACY POLICY supplements the information contained in Practice Depot, LLC’s (“PD”) Privacy Policy and it applies to all users and visitors of the Website who reside in the State of California. This notice complies with the California Consumer Privacy Act of 2018 as amended, hereto referred to as the “CCPA” which provides certain rights and specific notification information to the residents of California regarding data collection, use, and disclosure practices.
Categories of Personal Information: We collect information that identifies, relates to, describes, references, is reasonably capable of being associated with, or could reasonable be linked directly or indirectly with a particular consumer or device. The following is a list of categories of personal information as defined by the CCPA and the examples provided in the list below are those specifically defined in the CCPA.

- Information available from Government Records
- De-Identified or Aggregated consumer information
- Health and Medical Information covered by the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
- Personal information covered by certain sector-specific privacy laws such as the Fair Credit Reporting Act (FCRA) or California Financial Information Privacy Act (FIPA)
Sources of Personal Information: We obtain the categories of personal information listed above from several sources, including but not limited to:
- Directly from you when using one of our Websites or when you provide it to create an account.
- Vendors who submit your information to one of our affiliates for financial or insurance assistance.

- Right to Opt-Out: You have a right to instruct PD and their affiliates to not sell your personal information. To submit the Opt-Out request, please submit all information to PD via the contact methods listed in the Contact Us section of this agreement.
- Right to Correct: You have the right under the CCPA and other regulations and laws to correct any inaccuracies regarding your personal information that we have or may have about you. We will use commercially available and reasonable efforts to correct any inaccuracies that you request about the personal information we may have, considering the nature of the personal information and the purpose of the use before proceeding.
- Right to Delete: You have a right to request that your personal information be deleted with certain exceptions. Once we have received a formal request to delete, we will delete and instruct our service providers, contractors, and third parties to delete your personal information from the records, subject to the exceptions. We may deny the deletion request if retaining such information is necessary for us to:
- Complete the transaction for which we collected the personal information, provide a good or service that you requested, take actions reasonably anticipated within the context of our ongoing business relationship with you, or otherwise perform our contract with you.
- Comply with any legal obligation.

Only you, or a person registered with the California Secretary of State that you authorized to act on your behalf, may make a verifiable consumer request related to your personal information. You may only make a verifiable consumer request for access or data portability twice within a 12-month period. The verifiable consumer request must:
- Provide sufficient information that allows us to reasonably verify you are the person we have collected the information on or an authorized representative of the person we collected personal information on.
- Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond to it.
Response Timing and Format: We endeavor to respond to a verifiable consumer request within 45 days of its receipt. If we require more time, we will inform you of the reason and extension period in writing. If you have an account with us and we can communicate through the Website, we will deliver our written response to that account. If we are unable to deliver the response through the Website or you do not have an account with us, we will deliver our written response by mail or electronically, at your option. Any disclosures we provide will only cover the 12-month period preceding the verifiable consumer request’s receipt. The response we provide will also explain the reasons we cannot comply with a request, if applicable. For data portability requests, we will select a format to provide your personal information that is readily useable and should allow you to transmit the information from one entity to another entity without hindrance. We do not charge a fee to process or respond to your verifiable consumer request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request.
Changes to This Privacy Policy: This Privacy Policy is effective as of January 1, 2025, and will remain in effect except with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page. We reserve the right to update or change our Privacy Policy at any time and you should check this Privacy Policy periodically for updates. Your continued use of the Website after we post any modifications to both the Privacy Policy and California Privacy Policy will constitute your acknowledgment of the modifications and your consent to abide and be bound by the modified Privacy Policy and California Privacy Policy. If we make any material changes to this Privacy Policy or the California Privacy Policy, we will notify you either through the email address you have provided us, or by placing a prominent notice on the Website.
